Please fill in the information that will appear on your pass at the event in November. The information given will be transmitted as is on the passes, so please check carefully for accuracy and spelling before sending the form. The deadline for sending us your customization is October 1. After this date, no modifications will be accepted. If we do not receive your form, generic passes reading 'Expert PME' will be available at the registration desk during the event.
* If you have a 10x10 advice space: You are entitled to 6 passes.
* If you have a 10x20 advice space: You are entitled to 12 passes. You will therefore need to complete and return this form 2 times.

You can pick up your personalized passes during setup on November 16.

If there are any staff changes after you have submitted your customization information, generic passes will be available at the registration desk during the event. Deadline for producing customized passes: October 1.
