Customization form
passes for my on-site team

Please fill in the information that will appear on your pass at the event in November. The information given will be transmitted as is on the passes, so please check carefully for accuracy and spelling before sending the form. The deadline for customization is August 30. After this date, no modifications will be accepted. If the form is not received, generic passes will be sent to you for the November event, indicating "Expert PME".
* If you have a 10x10 consulting room: You are entitled to 6 passes.
* If you have a 10x20 consulting room: You are entitled to 12 passes. You will therefore need to complete and return this form 2 times.
